These forms are designed to help ensure that Spanish-speaking patients clearly understand their treatments, risks, and expectations—supporting both patient safety and compliance standards.

Providers may use these consents as part of their standard intake and treatment workflow when appropriate. As with all documentation, it is the provider’s responsibility to confirm that patients fully understand the information presented and to answer any questions prior to treatment. 

Take a photo or screenshot of the signed consent, and upload it to the patient chart for proper documentation.
