Types of Notes in the EHR

In the EHR, various types of notes are utilized to ensure comprehensive patient care and effective communication. Each note type serves a specific function.


 
Physician notes
Clearing physicians (the individual that processes a GFE for a patient) can leave notes on the service visit, documenting important details and observations.
For providers on the PortraitLegacy model these notes must be marked as read and acknowledged for the provider's payment to be released. They can be found on the feedback tab. 
Visibility: Only visible to admins and the practitioner.
 
 

 
Sticky Notes
Sticky notes can be found in a patient's profile for both admins and doctors to leave comments on that particular patient.
Visibility: Visible to admins, physicians and practitioners. 
 

 
Practitioner Notes
Practitioner notes can be found on a patient's profile and allow providers to add additional observations or comments regarding the patient. These notes are not visible to the patient, ensuring that sensitive or internal considerations remain confidential. Practitioner notes serve as a useful tool for enhancing communication among the healthcare team. 
Visibility: Visible to admins, physicians and practitioners. 
 
 

 
Customer Notes
Customer notes can be added at the end of a service visit (after step 10: charting) and provide insights specific to that visit or general information about the patient. These notes are accessible to patients, allowing them to stay informed about their care. 
Visibility: Visible to patients, admins, physicians and practitioners.
 
 


Internal Notes
Internal notes can also be found at the end of a service visit and allow practitioners to leave comments, clarifications or additional details on that specific service visit. They are only visible to admins and the practitioner, ensuring that sensitive information about the patient or the service visit remains confidential.
Visibility: Visible to admins, physicians and practitioners.
