[Legacy/Luxe] Medical director's pay set up

Medical directors for providers on the Luxe and Legacy models are paid the 15th or following business day after the month has ended.

If it's their first time, they'll be notified by Portrait's Accounting team to setup their payment account using a very quick process a day or two before the 15th after their first month.
