We've noticed that documentation for these visits can sometimes be challenging, and we’re here to help.
To make things easier, we’ve created a follow-up note template to help you consistently and thoroughly document patient visits. Here’s how to incorporate it into your workflow on a go-forward basis:
For Existing Weight Loss Patients:
- At each follow-up visit, print and complete the follow-up form.
- Attach the completed form to the patient’s chart.
- Repeat this process for every visit to ensure ongoing, accurate documentation of patient progress and care.
We are also sending a laminated version of this follow-up template to your office for additional convenience. You can fill it out during the visit, take a quick photo, and upload it to the patient’s chart immediately.

Important Reminder:
If you are dispensing sterile products to new weight-loss patients, specific documentation is required. You can access the template for that here. Additionally, all new weight-loss patients should have the follow-up form completed at each check-in to ensure consistent and thorough documentation.
