If the EHR is not accessible due to a technology outage or if you are unable to create a service visit for any other reason, here is what you should do.
1. Please request the client signs a paper consent form. These can be found in the binder provided during your onboarding. If you are unable to locate these forms, please print one:

4. Record all necessary information in the Manual Service Visit Form
