      How to use the Evolus Reward Program

      This article will explain how Providers can sign up and utiliize the Evolus reward program when using Jeuveau.

      To sign up for the Evolus/Jeuveau rewards program please follow these steps: 

      1. Navigate to this page and select "Start" https://evolus.typeform.com/to/Rmy0Wa
      2. On the next page enter the name of your Evolus sales rep. 
      3. In step 2 enter "Portrait" as the National Account Name. 
      4. For step 3 enter the name of your business. 
      5. Next enter the ship to # for your location. This number should have been sent to you after your Evolus account was created. The number will always start with an F. If you don't know your account number please submit a ticket here. 
      6. In step 5 enter the first and last name for the person who owns or manages the facility. 
      7. In step 6 enter in your own email address and in step 7 enter in the email address for any staff member at your facility that you would like to check in patients. 
      8. In 1-3 business days you will receive a docusign form to your email. Once signed all of the emails will be added to the Evolus portal and each user will receive an email with a temporary login. 
      9. To login go to myevolus.evolus.com to check in patients 
      10. After 8 patient check-ins you will receive 1 reward vial. To redeem your reward vial please submit a ticket here. 

      FOR LEGACY PROVIDERS ONLY

      • Please note any discount given to a patient that falls under the suggested retail price of $12 will result in a deduction in pay. 
      • To give a discount to a patient please select "custom discount" from the drop down menu on the checkout page. Please keep in mind there is no field for Evolus discounts like there is for Alle Pay or ASPIRE. 
      • Scan the vial as normal. 
      • After checking in 8 patients, a reward vial will be ordered for you and tagged as "naive". If you choose to sell this vial any revenue generated will create a surplus that can be used to offset any deductions generated in the same month. For example:
        • If You give out 8 x $30 discounts for a total of $240 in the month of Nov. 
        • In the same month you receive your reward vial and sell 20 units from that vial at $12 per unit for a total of $240, also in the month Nov. 
        • The surplus generated will offset the discounts given. 
        • If the surplus does not fall in the same month as the discounts the Provider will be charged for the deduction.
