How to Retrieve Your CareCredit Login Information

Access Care Credit Provider Center

Go to the Care Credit Provider Center -  carecreditprovidercenter.com


Verify Your Username

On the Provider Center login page locate the “Forgot User name?” link under the USER NAME bar. Click the “Forgot User name” link. On the next page enter Portrait’s merchant ID and your email address. Care Credit will have you confirm your identity by sending a one time passcode via a voicemail or to your email address. Choose which method you prefer to receive the one time passcode, then press CONTINUE. Care Credit will send you the one time passcode via the method of your choice. Once received enter the 5-digit passcode into the designated area on the website. Care Credit will reveal your username.


Verify Your Password

On the Provider Center login page locate the “Forgot Password?” link under the PASSWORD bar. Click the “Forgot Password” link. On the next page enter your user name and Portrait’s merchant ID. Care Credit will have you confirm your identity by sending a one time passcode via a voicemail or to your email address. Choose which method you prefer to receive the one time passcode, then press CONTINUE. Care Credit will send you the one time passcode via the method of your choice. Once received enter the 5-digit passcode into the designated area on the website. You will be prompted to create a new password before being granted access into your account.


New Care Credit Password Rules

  • Must be between 8 and 20 characters
  • Must contain both upper and lowercase letters
  • Must contain at least 2 numbers
  • Cannot be the same as last 10 passwords
